Remote Access Information
Remote access refers to
access that utilizes a keyfob. This means access from home or from
an office not connected to the Memorial infrastructure. Below are
the steps to follow in order to obtain remote access to the Memorial
Healthcare System from the D.O.C.S. page utilizing a keyfob.
- In order to initiate an “Access From Home” request, physicians
should download and review the Physician Access Agreement form

Access Agrmt and then mail the signed copy to Mariellan Weaver,
Executive Assistant, Medical Staff Office, 615 N. Michigan Street,
South Bend, IN 46601 or fax to (574) 647-6691 or email mweaver@memorialsb.org.
- Once the Physician Access Agreement form is received, your training
status will be verified. Remote access to Cerner applications
(ie. Powerchart) cannot be obtained if your training is not current.
If training is needed, someone from the Medical Staff Office will
contact you.
- Once your training status is current, Information Systems will
establish your network account and issue a keyfob. A packet including
the keyfob, a letter containing your network account information,
and installation instructions will be sent by Mariellan Weaver
in the Medical Staff Office, 574-647-7920. She will mail the certified
packet to your office
